What helps when you refer
- The question you want answered, with tooth or implant numbers
- Available recent periapicals and bitewings, with dates
- Available periodontal charting and a history of scaling and root planing, including dates and response to treatment
- Medical flags such as anticoagulants, antiresorptives, diabetes, or smoking
- For implant cases, the restorative plan and who will restore
Communication and treatment planning
Treatment planning starts with your referral question, the patient's priorities, and any restorative plan. Include your preferred contact details with the referral so Dr. Smith can discuss the findings and treatment with you.
What your office can expect
- A written consultation report to your office within one to two weeks.
- A second report when active periodontal or implant treatment is complete.
- Periodontal maintenance usually alternates with your office at three-month intervals, unless you prefer another arrangement.
