What this website does not do

• It does not set cookies or use analytics, advertising, or social-media trackers.
• It does not load fonts, scripts, or images from other companies' servers.
• It has no contact form or account, so it does not collect your name, email address, or phone number. If you choose to email Dr. Smith, your message goes to his email account, not to this website.

The online health history

When you use the guided health history, your answers stay on the page in your own browser. They are not sent to the website, the offices, or anyone else, and they are not saved in your browser's storage. Leaving or refreshing the page clears them.

The completed PDF is created on your own device. You decide whether to print it, bring it to your appointment, or email it to drsmith@johnsmithdds.com. Mail to that address is stored encrypted with Proton Mail under a HIPAA business associate agreement. Email is protected in transit by standard encryption; if you prefer a fully encrypted message, ask for a secure message link and reply through it.

Technical logs

Like most websites, the service that hosts this site may keep standard technical logs, such as IP addresses and the pages requested, to keep the site secure and working. These logs are not used to identify or profile visitors.

Links to other websites

This site links to the practices where Dr. Smith provides care, to map directions, and to the health sources cited in each guide. Those websites have their own privacy policies.

Your dental records

Your dental records are kept by the practice where you are treated, under that practice's own policies and its Notice of Privacy Practices. Ask the office for a copy of its notice.
